What is trochanteric bursitis?

Trochanteric bursitis is inflammation of the bursa, a small fluid-filled sac that cushions the greater trochanter, the bony prominence on the outer side of the hip. Inflammation of this bursa causes pain on the outer hip.

Symptoms

Common symptoms of trochanteric bursitis include:

  • Pain on the outer side of the hip

  • Pain that worsens when lying on the affected side

  • Tenderness over the outer hip

  • Pain with stairs, prolonged standing, or walking

Causes and risk factors

Trochanteric bursitis can develop for a number of reasons, including:

  • Repetitive friction or overuse

  • Direct trauma to the hip

  • Underlying abductor tendon problems

  • Previous hip surgery

Treatment options for trochanteric bursitis

Trochanteric bursitis is generally managed successfully without surgery, particularly when addressed early.

Non-surgical treatment

Non-surgical treatment is the mainstay of management, and may include:

  • Rest and activity modification

  • Physiotherapy

  • Anti-inflammatory medication

  • Corticosteroid injections

The majority of cases resolve with non-surgical treatment alone.

Surgical treatment

Surgical treatment is uncommon and is generally reserved for persistent symptoms that do not respond to non-surgical management, particularly where there is an associated tear of the abductor tendons.

  • Abductor Repair — may be considered where trochanteric bursitis is associated with a tear of the hip abductor tendons that has not responded to non-surgical treatment.
